2013 APPLICATION FOR INTERNATIONAL TRAVELING FELLOW
TO THE AMERICAN SOCIETY FOR SURGERY OF THE HAND

Purpose: The American Society for Surgery of the Hand invites official IFSSH recognized Hand Societies to support a Traveling Fellow visit to the United States.

Process: The visiting fellow should be selected by a participating country’s Hand Society through a formal process.  The ASSH Traveling Fellowship Committee Chair will waive the qualifying fellow’s registration fee for attendance at the ASSH Annual Meeting and will assist the International Traveling Fellow in arranging visits to hand surgery centers in the U.S., should this be desired.  The visits will take place in the weeks following ASSH’s Annual Meeting, October 3-5, 2013 in San Francisco, California.  All travel, housing and meal costs will be the responsibility of the Traveling Fellow or his/her sponsoring country’s Hand Society.  This award is limited to one participant per hand society per calendar year.

• Application form (see next page)
• Traveling fellow’s CV
• Photograph of traveling fellow (needed for the brief presentation the recipient usually makes at the ASSH Annual Meeting)
